Bupropion: uses, licensing and how it differs by country

If you are in crisis, get help now. In Ireland and the UK, call Samaritans free on 116 123, any time. In Ireland you can also text 50808. In the US, call or text 988. If you or someone else is at immediate risk, phone 112 or 999 (Ireland and the UK) or go to an emergency department.

Bupropion is a prescription medicine that blocks the reuptake of noradrenaline and dopamine. It is often called an NDRI. In the US it is approved for depression, for preventing seasonal depression and, as a separate product, to help people stop smoking. In Ireland and the UK, the only bupropion-only medicine we found listed is Zyban. It is licensed for one thing: helping people stop smoking. We found no bupropion product licensed for depression in either country.

So the same molecule has a different job depending on where you live. Below we cover the licences, the evidence and the safety points. Whether it suits you is a conversation for your GP or prescriber.

What bupropion does in the brain

Most common antidepressants, including the SSRIs, work mainly on serotonin. We cover how SSRIs work separately. Bupropion is different. The UK and Irish product information describes it as a selective inhibitor of noradrenaline and dopamine reuptake, with minimal effect on serotonin reuptake. It does not inhibit monoamine oxidase.

The US label calls it a "relatively weak" inhibitor of noradrenaline and dopamine uptake. It also says its exact mechanism of action is unknown, as with other antidepressants.

That dopamine and noradrenaline profile is why it gets discussed alongside ADHD medicines. It is not classed as a stimulant, though, and it is not licensed for ADHD anywhere we checked.

What it is licensed for, by country

A licence sets out which conditions a medicine is approved for. Here is what we found in Irish, UK and US listings in November 2026.

NICE's depression guideline for England does not mention bupropion. Doctors can sometimes prescribe a medicine "off-label", meaning outside its licence. The UK regulator, the MHRA, says this may be justified when a prescriber judges it to be in the patient's best interest. It adds that the prescriber's responsibility may be greater when they do.

Bupropion for stopping smoking

This is the use with the strongest evidence. A 2023 Cochrane review pooled 50 trials with 18,577 participants. It found high-certainty evidence that bupropion raised long-term quit rates compared with placebo or no medicine (risk ratio 1.60, 95% CI 1.49 to 1.72).

The same review is clear that bupropion is not the strongest option. It was less effective than varenicline and than combination nicotine replacement therapy. It performed about the same as a single form of nicotine replacement. People taking it were also more likely to drop out of trials because of side effects. NICE's guidance reflects this. It lists bupropion among the options that are "less likely" to work than varenicline, cytisinicline, combination NRT or nicotine e-cigarettes. NICE also says not to offer bupropion to people under 18.

Our piece on smoking and the brain covers what quitting changes.

Bupropion for depression: what the evidence shows

In the US, bupropion has been an approved antidepressant since the 1980s. A large 2018 network meta-analysis in The Lancet compared 21 antidepressants. It included 522 trials and 116,477 participants. All 21, bupropion included, did better than placebo on response.

Three caveats from that paper apply to bupropion:

One comparison is worth knowing about. Two identical 8-week trials compared bupropion XL with escitalopram and placebo in 830 adults. On the main depression scale, bupropion XL did not separate from placebo at the usual statistical threshold. Escitalopram did, in one of the two studies and in the pooled data. The two drugs did not differ significantly from each other.

That does not mean bupropion fails as an antidepressant. It does mean its evidence base is less certain than for some alternatives. For where antidepressants sit alongside therapy and exercise, see depression: what actually works, in order.

Seasonal depression. The US approval for seasonal affective disorder is for prevention. It rests on three placebo-controlled trials in adults with a history of autumn-winter depression. Our guide to daylight and mood covers the non-drug side.

Bupropion for ADHD: off-label everywhere

Bupropion is not licensed for ADHD in Ireland, the UK or the US. It is sometimes used off-label when stimulants do not suit someone.

The evidence is limited. A 2017 Cochrane review of adults found six trials with 438 participants, all lasting 6 to 10 weeks. Bupropion reduced ADHD symptom severity compared with placebo (standardised mean difference −0.50, 95% CI −0.86 to −0.15). The authors rated the evidence as low quality. They said further research is very likely to change the estimates.

A 2018 network meta-analysis in The Lancet Psychiatry found bupropion beat placebo on clinician ratings in adults (SMD −0.46, 95% CI −0.85 to −0.07). That confidence interval comes close to zero. A larger 2025 analysis of adult ADHD treatments found only stimulants and atomoxetine improved symptoms on both self-rated and clinician-rated scales. Bupropion was not among them.

NICE's ADHD guideline does not include bupropion in its recommended medicines. It says medicines outside its list should not be offered without advice from a tertiary (specialist) ADHD service. Here is how the licensed ADHD medicines compare.

Seizure risk and what the labels say about dose

Every bupropion label we read carries a prominent seizure warning. The risk is dose-related.

Each label sets a maximum. These are the regulators' ceilings, not dosing advice.

The labels also say tablets must be swallowed whole, because crushing or chewing them can raise seizure risk.

Who should not take it. The Zyban information lists, among others:

Other risk factors call for extra caution. These include head injury, heavy drinking, diabetes treated with insulin or tablets, and medicines that lower the seizure threshold.

Side effects

The Zyban information lists insomnia as very common, affecting more than 1 in 10 people. Common effects include:

Raised blood pressure can occur, sometimes severely. Labels advise checking it before and during treatment. Weight loss is more common than gain. In US depression trials, 14% on 300 mg a day lost more than 5 lb, against 6% on placebo.

Rarer serious effects include severe skin and allergic reactions. Psychotic and manic symptoms have been reported, mainly in people with a known psychiatric history. Bupropion may also unmask Brugada syndrome, a rare inherited heart rhythm condition.

Suicidal thoughts. The US labels carry a boxed warning. Antidepressants raised the risk of suicidal thoughts and behaviour in children, teenagers and adults under 25 in short-term trials. Low mood is also a recognised part of nicotine withdrawal. If your mood drops or you have thoughts of self-harm, contact your prescriber straight away. Or use the numbers at the top of this page.

Why it is linked to fewer sexual side effects

Sexual side effects are common with SSRIs. Bupropion has a better record here, with some caveats.

The likely reason is the mechanism. Bupropion has minimal effect on serotonin reuptake. One review notes it is "generally thought" that raised serotonin dampens sexual function. That is a working explanation, not a settled one.

Interactions to ask about

The main interacting groups in the labels are:

Bupropion can also cause a false positive for amphetamines on some urine drug screens. And stopping smoking can itself raise blood levels of some medicines, such as clozapine and theophylline, so tell your prescriber you are quitting.

What to ask your prescriber

FAQ

Can I get bupropion for depression in Ireland or the UK?

It is not licensed for depression in either country. Off-label prescribing is a decision for the prescriber. If it is suggested, ask them to explain why and what the alternatives are.

Is bupropion a stimulant?

It is not classed as one. Its labels describe it as an aminoketone antidepressant. The US label notes some stimulant-like effects in animal studies. The UK and Irish information says human studies show low abuse potential.

Does bupropion cause weight gain?

In US depression trials, weight loss was more common than weight gain. Bupropion is also one half of a licensed weight-management combination, Mysimba (Contrave in the US).

Is Zyban the same as Wellbutrin?

They contain the same active ingredient, bupropion hydrochloride. They are different products with different licences, and you should never take two bupropion products together.
