Creatine and the Brain: What the Cognitive Research Actually Shows

Creatine Monohydrate is one of the most heavily researched supplements in existence — but almost all of that research has been about muscle. In the last few years, a newer question has picked up momentum: does creatine do anything for the brain? Here's what the actual trial evidence says, including the parts that don't flatter a sales pitch.

Why the brain, of all places

Around 5% of the body's creatine pool sits in the brain, where it plays the same basic role it plays in muscle: helping regenerate ATP, the cell's short-term energy currency. Brain tissue is metabolically expensive to run, especially under fatigue, sleep deprivation, or mental exertion — which is the biological rationale researchers have used to test whether topping up creatine stores helps cognitive performance the way it helps physical performance.

What the meta-analysis actually found

A 2024 systematic review and meta-analysis in Frontiers in Nutrition pooled 16 randomized controlled trials (492 participants) testing creatine supplementation against cognitive outcomes.1 The results were mixed, not uniformly positive:

  • Memory showed a real, moderate-certainty improvement (effect size 0.31).
  • Processing speed and attention both improved, but the certainty of evidence behind those results was rated low.
  • Overall cognitive function and executive function showed no meaningful improvement.

The benefit also wasn't evenly distributed: creatine performed better in adults aged 18–60 and in people with existing medical conditions than in healthy adults over 60. The review's own authors were explicit that larger, more rigorous trials are still needed before treating these findings as settled.

Why EFSA said no

This is the part most brands leave out. In late 2024, the European Food Safety Authority formally evaluated a proposed health claim linking creatine supplementation to improved cognitive function — and rejected it, concluding the evidence wasn't sufficient to substantiate the claim.2 That's a real regulatory body applying a stricter bar than a meta-analysis alone, and it's a useful check on how far the current evidence can honestly be stretched.

So where does that leave things

Creatine's cognitive research is real, growing, and worth watching — the memory finding in particular is more than noise. But it hasn't yet cleared the bar for a formal cognitive-function claim, and the certainty behind several of the positive findings is still low. That's exactly the kind of nuance we think matters: promising is not the same as proven, and we'd rather say that plainly than round it up.
