Ozempic vs Wegovy vs Mounjaro vs Zepbound: What's the Difference?

There are really only two molecules here. Ozempic and Wegovy are both semaglutide — the same active ingredient, made by the same company, approved for different conditions. Mounjaro and Zepbound are both tirzepatide — again the same molecule, different brand name, different approved use. Understanding which is which makes the whole landscape much simpler to navigate.

Why does the same molecule have two brand names?

Drug manufacturers often seek approval for the same active compound under different brand names for different clinical indications. This is partly a regulatory and commercial process: the approved doses, labelling, and clinical data package submitted to regulators differ between the diabetes and weight-loss indications, even though the molecule itself is the same.

In practice, this means Ozempic and Wegovy are both semaglutide — but the approved maximum doses and titration schedules for Wegovy (the weight-loss version) are higher than those for Ozempic (the diabetes version). Similarly, Zepbound's weight-loss dosing reflects its own approved schedule.


What's the difference between semaglutide and tirzepatide?

Semaglutide (Ozempic / Wegovy) is a GLP-1 receptor agonist — it mimics one gut hormone that signals fullness and slows gastric emptying. Tirzepatide (Mounjaro / Zepbound) is a dual GIP/GLP-1 agonist — it targets two receptors. In head-to-head clinical trial data (the SURMOUNT-5 trial), tirzepatide produced greater average weight loss than semaglutide, though individual responses vary considerably.


Which one is right for me?
